Welcome to CVC

Please arrive 30 minutes before your first appointment to complete new patient paperwork. Bring a photo ID and all current insurance cards. Wear comfortable, loose-fitting clothing, as you may need to partially undress for examination of areas like your legs. Come prepared with a list of current medications, supplements, any past vascular testing/procedures, a referral form (if applicable) and a description of your symptoms so we can best evaluate your situation.

Payment of all co-pays, estimated co-insurance, and deductible amounts are required at your visit. Copays will be collected at check-in for your convenience.

It’s best to wear comfortable clothing to your appointment. You will be asked to change into a gown for an examination. We also ask that you bring all of the medications you are currently taking. After being evaluated by the physician or nurse practitioner, it will be determined if tests are necessary.

Depending upon medical findings and any symptoms that you are experiencing, some diagnostic tests may be ordered, including blood work. Most of the testing is done in our CVC offices. At times, tests may be performed at an outside facility or local hospital.
